Why does my thumb hurt when I pour warm water on it?

Hello. I recently discovered that when I pour warm water on my thumb it starts to hurt and the pain goes thru the whole arm. It only happens when water is poured on it but when I am not showering or washing my hands my arm feels fine.

My Mother age 78 yers x ray report There is presence of facyure of neck of right femur with overridding. Osteoporosis of bones under vision noted. Lumber spine shows mild-to-moderate spondylotic changes. IVD spaces are preseved. No abnormal soft tissue opacity seen.
